The American Dental Association, the World Health Organization, and the American Academy of Pediatrics all endorse community water fluoridation at recommended levels. Numerous systematic reviews have concluded that fluoridated water reduces tooth decay in both children and adults, with particular benefit for populations with limited access to regular dental care.
Topical fluoride — found in toothpaste, professional treatments, and prescription rinses — is similarly well-supported. Most dental professionals consider a fluoride toothpaste to be a non-negotiable component of a daily oral hygiene routine, and the evidence for its protective effects is extensive.
Where Legitimate Concerns Enter the Picture

Dental fluorosis is one acknowledged consequence of excessive fluoride exposure during early childhood, when teeth are still developing. Current guidelines from the American Academy of Pediatric Dentistry recommend that children under three use only a rice-grain-sized amount of fluoride toothpaste, and children three to six use no more than a pea-sized amount, precisely to manage this risk.
Skeletal fluorosis is a condition associated with very high fluoride intake over extended periods. It is primarily documented in regions of the world where naturally occurring fluoride in water sources far exceeds levels used in U.S. municipal systems. At the concentrations used in American water fluoridation — currently recommended at 0.7 milligrams per liter — skeletal effects have not been demonstrated in the research literature.
Neurological concerns represent the most contested area of the current debate. A 2020 report from the National Toxicology Program and a subsequent 2024 systematic review drew attention by suggesting a possible association between higher fluoride exposure and modest reductions in children's IQ scores in some studies. These findings generated significant media coverage and contributed to a federal court ruling that prompted the Environmental Protection Agency to reconsider its risk assessment of water fluoridation. It is important to note that many of the studies cited in these reviews were conducted in regions with fluoride levels considerably higher than those used in U.S. water systems, and that causation has not been established. The research is ongoing, and professional organizations have called for more rigorous investigation rather than immediate policy changes.
Why Some Communities Have Chosen to Remove Fluoride
In recent years, a small but notable number of American municipalities have voted to end water fluoridation programs. Their decisions reflect a combination of factors: evolving public attitudes toward government-administered health interventions, concerns about individual choice, and in some cases, the precautionary principle — the idea that when uncertainty exists, reducing exposure is prudent.
Public health officials and dental organizations have generally opposed these decisions, citing the disproportionate impact on lower-income communities where access to regular dental care and fluoride toothpaste may be inconsistent. Research following communities that have ended fluoridation has in several instances documented increases in childhood tooth decay rates in subsequent years.
Making Sense of This for Your Own Health

First, topical fluoride use is supported by strong, consistent evidence and involves far lower systemic exposure than ingested sources. Using a fluoride toothpaste twice daily remains a well-justified recommendation for the vast majority of patients.
Second, age matters significantly. Young children require careful supervision of fluoride toothpaste use to minimize swallowing. Adults and older patients, by contrast, face no meaningful risk from topical fluoride at standard concentrations and stand to benefit considerably from its protective effects.
Third, individual risk factors should guide professional recommendations. Patients with high cavity rates, dry mouth, or other conditions that increase decay risk may benefit from prescription-strength fluoride products. Those with lower risk profiles may need less aggressive intervention.
Finally, open communication with your dental provider is essential. The Broader Principle
